We expect care providers to:
- Treat patients in a skilled and caring manner.
- Inform the patient about the options available regarding treatment and care and obtain the informed consent of the patient.
- Seek permission from the patient to communicate with a family member, friend or outside care provider about his/her health status and plan of care, when necessary and appropriate.
- Maintain confidentiality.
- Inform the patient of an estimated time of discharge as soon as possible following admission.
- Facilitate access to community services, if necessary, prior to discharge to ensure a continuity of care.
We expect patients to:
- Share relevant information with care providers.
- Identify one contact person, (a family member or care provider) who we can inform about his/her health and plan of care – when necessary or appropriate.
- Share information with respect to substitute decision makers regarding personal and financial care – as necessary.
- Accept responsibility for decisions he/she makes about personal care and treatment.
We expect patients, families and care providers to:
- Respect each other’s values, beliefs, privacy and time.
- Contribute to and maintain a clean and safe environment.
- Share in the responsibility for appropriate use of health care services.
- Respect and comply with the hospital policies, rules and legislated requirements.
- Communicate and behave with respect towards each other.
