‘Living With Diabetes’ - A 2-Day ‘Daytime’ Session

This session is for people with a diagnosis of Type 2 diabetes There are two parts to this program. First, you will be seen for an assessment by the nurse and the dietitian. The first visit includes a clinical assessment and a review of the food, lifestyle and monitoring basics for good blood glucose control. Then, you will attend a two day group education session. The 2-day session includes talks from a doctor, a nurse, a dietitian, a pharmacist and a foot care nurse specialist. You will learn about diabetes, how your medications work, how and when to check your diabetes, how to care for your feet, how food affects blood glucose and how to make the best food choices for good blood glucose control and heart health.

‘Living With Diabetes’ - A 3-Night ‘Evening’ Session

This session is for people with a diagnosis of Type 2 diabetes There are two parts to this program. First, you will be seen for an assessment by the nurse and the dietitian. The first visit includes a clinical assessment and a review of the food, lifestyle and monitoring basics for good blood glucose control. Then, you will attend three evenings of group education given by a nurse and a dietitian. You will learn about diabetes, how and when to check your diabetes, how to care for your feet, how food affects blood glucose and how to make the best food choices for good blood glucose control and heart health.
